Care Coordination
Care coordination services help older adults and caregivers determine what their specific needs are and what services are available to meet those needs.
Community-based services may include those that are funded by the state and federal government as well as those that a person can purchase with their own resources.
Eligibility: older adults over the age of 60; must reside in one of the CCU contracted service areas (Lyons, Riverside, South Proviso, Leyden or Norwood Park Townships).
How to access this service: Call 708-354-1323 ext. 215 during regular business hours (M-F 8:30am – 4:30pm) and ask to speak with an Information and Assistance Specialist to begin the intake process/application for in-home services. What you can expect: Following intake, you will be contacted by a certified Care Coordinator to schedule a comprehensive in-home assessment. During the assessment you will discuss what kinds of daily activities you do on your own and those that require assistance. The Care Coordinator can then identify services that could help you to continue living in your own home or community.
Community Care Program
The Community Care Program helps older adults who might otherwise need nursing home care to remain in their homes by providing in-home and community-based services.
It aims to assist older adults in maintaining their independence and providing cost-effective alternatives to nursing home placement. Services include: adult day services, in-home services, emergency home response services, and automated medication dispenser services.
Adult Day Service Offers direct care and supervision in a community-based setting which provides personal attention and social, physical, and emotional well-being in a structured setting. ADS provides respite for family caregivers, especially those who are employed outside the home, and socialization for isolated adults. Services include: Arranging transportation; Assistance with daily meals; Assistance with activities of daily living; Support with health-related services
In-Home Service Offers non-medical support by supervised homecare aides who have received specialized training in the provision of in-home services. Services include: Accompany / Transportation to medical appointments and errands such as shopping; Assisting with self-administered medication; Following a written special nutrition diet plan; Moderate home maintenance; Non-medical personal care tasks; Performing meal preparation; Routine light housekeeping
Emergency Home Response Service Offers you a 24-hour emergency communication link to assistance outside your home. This service is provided by a 2-way voice communication system consisting of a base unit and an activation device worn, automatically linking to a professionally staffed support center. The support center assesses the situation and directs an appropriate response whenever activated.
Automated Medication Dispenser Service is available to eligible participants who need medication reminders to improve medication adherence. Offers a portable, mechanical system for individual use that can be programmed to dispense or remind to take oral medications through auditory, visual, or voice reminders.
Eligibility: Adults age 60 or older; determined physically in need of services and supports based on the comprehensive assessment tool; resident of the State of Illinois; either a U.S. citizen or an eligible non-citizen within the specific categories; meet the asset requirements; and agree to apply, and if found eligible, enroll for Medicaid benefits on an annual basis.
Must reside in one of the CCU contracted service areas (Lyons, Riverside, South Proviso, Leyden or Norwood Park Townships).

What you can expect: Following intake, you will be contacted by a certified Care Coordinator to schedule a comprehensive in-home assessment. If eligible for services and you decide to enroll, your Care Coordinator will make all the arrangements in linking you to approved direct service providers and they will continue to follow up with you and be available to you to provide any assistance you might need while receiving services.
Chore Housekeeping
Chore is a home maintenance program that helps older adults having difficulty with one or more daily activities complete tasks such as light housekeeping, laundry, and shopping for essential groceries and medications.

What you can expect: Following intake, you will be contacted by a certified Care Coordinator to schedule a comprehensive in-home assessment. Chore services are delivered one or two times per month, for a total of four hours per month, based on need and availability.
Additionally, this program has a suggested donation of $1/hour. The inability to donate does not exclude you from the program.

HomeMeds is a free review of all your medications by a pharmacist.

If you answer YES to some or all of these questions, you may benefit from a review:
- You see multiple doctors
- You fill prescriptions at multiple pharmacies
- You have recently experienced a fall, dizziness, or unusual confusion
- You have recently been to the ER or hospital
- You take blood thinners or diabetes medications
- You take over-the-counter drugs or supplements
- You take medications to help you sleep

What you can expect: Following intake, you will be contacted by a certified Care Coordinator to schedule a comprehensive in-home assessment. During the visit, your Care Coordinator will prepare a list of all your medications, including any over-the-counter drugs or supplements to be reviewed by a specially trained pharmacist. If any problems are found, the pharmacist will work with you and your doctors to make sure that you are only taking the medications that are right for you.
Transitional Care – Choices for Care
Under the Choices for Care Program, individuals considering nursing facility admission are screened by Care Coordinators and informed about their long-term care options, including home and community-based service options.
This provides individuals with the information they need to make an informed choice about their options for long-term services and supports and prevents a person from entering a facility when their needs may be met at home.
Eligibility: Older adults age 60+ and individuals with disabilities age 18-59 who are considering nursing facility admission.
Must be currently hospitalized or reside in one of the CCU contracted service areas (Lyons, Riverside, South Proviso, Leyden or Norwood Park Townships).
How to access this service: Screenings most commonly occur in hospitals prior to discharge but can also occur in nursing facilities after admission or in an individual’s home in the community prior to admission to a supportive living or nursing facility. An individual or family member can either call ACC directly at 708-354-1323 ext. 215 during regular business hours (M-F 830-430) and ask to speak with an Information and Assistance Specialist to begin the intake process or they can request that hospital/facility staff reach out on their behalf. What you can expect: A Care Coordinator will assess the person’s needs and goals and determine their eligibility for long-term care services. The individual will be asked about their abilities to perform daily activities, such as personal care, preparing meals, shopping, housework and whether they currently have any assistance with these activities. Based on the assessment, the Care Coordinator will provide the individual with information to assist them with choosing the services that best meet their needs.
