The Community Resource Network (CRN) is a collaborative initiative comprising a diverse array of local businesses, nonprofit organizations, and volunteer agencies. This collaboration is committed to enhancing the quality of life for older adults by streamlining access to community-based services, particularly for those transitioning from acute or post-acute care settings. The CRN recognizes the unique challenges older adults face in these situations and is dedicated to creating tailored solutions that address any gaps in support services. By fostering partnerships among various stakeholders, the network aims to ensure a smooth and safe reintegration into the community. This includes facilitating essential resources and services and providing a supportive framework that empowers older adults to return home with confidence and the assistance they need to thrive. Through collaboration and innovation, the CRN strives to improve older adults’ overall well-being, helping them maintain their independence and enhance their quality of life.
