What you can expect: An I&A Specialist will briefly assess the situation and needs of the person, provide information and resources that support their range of needs, and link individuals to available opportunities and services.
Benefits Assistance
Information and counseling on Medicare – how to apply, available programs, and other public benefit assistance programs.

Counseling on (review and compare)/assistance with applying for Medicare benefits including Part D, low-income subsidies (Extra Help), Medicare Savings Programs (QMB, SLMB, SLIB, QI) and other public benefit assistance programs like Medicaid, Supplemental Nutrition Assistance Program (SNAP), Benefits Access (Seniors/Persons with Disabilities Ride Free and License Plate discounts), and Property Tax Exemptions (Homeowner, Senior Citizen Homestead and Senior Freeze).
Eligibility: Older adults over the age of 60 and adults with disabilities who are 18-59 years old; must reside in the service area
How to access this service: Call 708-354-1323 during regular business hours (M-F 8:30am – 4:30pm) for an appointment with a Senior Health Insurance Program (SHIP) counselor. Can also email 24/7 at info@agingcareconnections.org
What you can expect: Caring and knowledgeable certified SHIP Counselors to provide information, help navigate or apply for any public benefit assistance program.
Options Counseling
A person-centered, interactive, decision-support process whereby individuals receive assistance in their efforts to make informed long-term support choices specific to their own preferences, strengths, and values.

Eligibility: Older adults over the age of 60 and adults with disabilities who are 18-59 years old; must reside in the service area.

What you can expect: A personal interview, assistance with identifying choices available including personal, public and private resources, a facilitated decision-support process, assistance as requested and directed in developing an action plan, linkages to services if requested and follow-up.
Transportation
Connection to and assistance in making travel arrangements or applying for medical and non-medical transportation services in your community.

Eligibility: Older adults over the age of 60; additionally, all transportation service providers have their own eligibility requirements, unique application processes, and geographical boundaries; must reside in the service area

What you can expect: A comprehensive review /information on transportation providers in your area that may include township and village transportation programs, hospital/medical transportation, volunteer transportation and/or assistance in applying for a reduced fare or ride free pass valid on CTA, Metra and Pace lines.
