Aging Well Neighborhood (AWN)

Aging Well Neighborhood brings together older adults, businesses, healthcare and social services to create a “Health Neighborhood” that promotes life-long wellness, addresses chronic conditions, offers lifelong learning opportunities, and helps build an age-friendly community.

Eligibility: Older adults over the age of 60

How to access this service: To learn more, call 708-354-1323 during regular business hours (M-F 8:30am – 4:30pm) and ask to speak with a member of the AWN Team or by emailing us 24/7 at AWN@agingcareconnections.org. Healthy Aging Programs and Resources including workshops, seminars, expert speakers, fitness classes, preventative screenings, chronic disease management reference materials, health and wellness materials, and fitness guides for older adults, their caregivers, and their families; in-person, hybrid and remote programming; some programs offered in Spanish or bilingually.

Aging Well Community Teams

Aging Well Community Teams are driven by local teams of older adults, businesses, non-profits, and some local government staff and officials.

They work to build a culture of aging well in their individual communities by hosting educational seminars, social activities, health activities, and monthly gatherings. They make decisions/offer programs based on the identified needs, preferences and interests of the older adults living in each specific community.
